When a need for a specific medication is urgent, included is a formulary prescription card which you will take to your local pharmacy. The WellDyne Network has over 65,000 participating pharmacies nationally, including independent neighborhood options.
Includes over 125 of the most commonly used acute medications for a FREE 15-day supply. Each urgent medication may only be filled (2) times within a 180-day period.
How to use: Pick up your FREE 15-day supply of an acute medication listed on the Acute Formulary at participating pharmacies.
Maintenance Medications FREE 90-Day Supply
400+ Different Medications
Included are over 400 chronic medications in this formulary and are purchased as a 90-day supply through a dedicated 800 number from our national mail-order pharmacy. Chronic medications prescribed by your doctor assist in your maintenance of good health.
Maintenance medications are those your doctor prescribes for chronic conditions which are taken regularly for an ongoing period. Examples of chronic medications are high cholesterol, heart, and diabetic disease etc. (refer to included list of chronic medications). $9.00 service fee for mailing.
How to use:
- Ask your doctor to write a prescription for the number of days your plan allows (for example, 90 days).
- Your doctor can submit your prescription:
- Electronically: E-prescribe to WellDyne Home Delivery. This is the quickest way to fill your prescription.
- By FAX: 1-888-830-3608. Only prescribers may fax prescriptions to a pharmacy.
- By Phone: Call our Customer Service Department at 888-479-6570.
- By Mail: Send prescription with patient's address and date of birth to WellDyne, PO Box 90369, Lakeland, FL 33804.
- Note: You do not go to your local pharmacy for this service. The medications will ship directly to you.

Rx Savings Card
When your medication is not included as part of this formulary, included with this program is a valuable Rx Savings Card. The card can be used at over 65,000 national chain and independent pharmacies for generic medications providing up to 55% savings, and brand medications savings up to 15% savings.
How to use: Present this card to your local pharmacy who will confirm eligibility and apply savings to reduce the cost of your medication. You will pay the reduced cost to the pharmacy when you pick up your medication(s).
Brand-Name Savings
Save up to 80% on your brand-name medications and up to 45% on insulin* medications that are not ReliOn through our international mail-order program. Ordering your discount medications is convenient, easy, and secure.
